Here at Holy Ghost Community Health Care: we aim to help the service users to improve their health and regaining thier independence as much as possible by actively providing quality client centred care, measuring through risk assessments, as agreed with other multi- disciplinary (MTD) teams, to achieve a successful client wellbeing outcome. Again, we seek to provide round the clock care and support, to the elderly, adults with mental health issues, alcohol dependant, homeless individuals, learning and physical disabilities, isolated individuals, HIV/AIDS/ CANCER/ COVID by measuring symptoms and to individuals with terminal illness and individuals recovering from illness, children and families with autism, learning and physical disabilities.
